Longevity has a marketing problem. The internet is full of supplements, peptides, cold plunges and red-light panels promising to add years to your life, most of them backed by a single mouse study and a confident influencer. Meanwhile the single best-evidenced healthspan intervention we have is the one nobody can sell you in a subscription box: a good night's sleep. Sleep is not downtime. It is the active maintenance shift during which your brain clears waste, your hormones reset, your immune system recalibrates and your cardiovascular system gets a nightly break. Shortchange it chronically and you quietly raise your risk of nearly every major disease of aging at once.
Sleep is repair time, not the off switch
The old intuition that sleep is simply the brain powering down has not survived contact with modern neuroscience. During deep, slow-wave sleep, growth hormone is released in pulses that drive tissue and muscle repair; memories are consolidated; appetite and glucose-regulating hormones are reset; and — most strikingly — the brain runs a literal cleaning cycle. The discovery that ties all of this to aging is the glymphatic system, a macroscopic waste-clearance network that uses perivascular channels formed by astroglial cells to flush soluble proteins and metabolites out of the central nervous system. According to PubMed, a review of this system notes that it operates mainly during sleep and is largely switched off during wakefulness, which suggests the very need for sleep may reflect the brain's requirement to enter a state in which it can dispose of potentially neurotoxic waste, including β-amyloid.[1]
The experimental backbone for that idea is striking. According to PubMed, a 2013 study in Science using real-time imaging in live mice found that during natural sleep (or anesthesia) the interstitial space in the brain expanded by roughly 60%, dramatically increasing the convective exchange between cerebrospinal fluid and interstitial fluid — and that this surge in flow increased the clearance rate of β-amyloid during sleep.[2] In plain terms: the plumbing that washes out one of the proteins most associated with Alzheimer's disease runs on the night shift. Less deep sleep means a shorter, less effective cleaning cycle. That single mechanism reframes the stakes of a chronically bad night from "I'll be tired tomorrow" to "I am skipping maintenance the brain cannot easily do at any other time."
What the long-term mortality data show
When researchers track large populations over years and plot sleep duration against the risk of dying, they do not get a straight line. They get a U. According to PubMed, a systematic review and dose-response meta-analysis of prospective cohort studies found U-shaped associations between sleep duration and all-cause mortality, total cardiovascular disease, coronary heart disease and stroke, with the lowest risk consistently observed at around 7 hours per day. For all-cause mortality, the pooled relative risk rose about 6% per hour of sleep lost below 7 hours, and about 13% per hour gained above it; cardiovascular and stroke risk followed the same bowl-shaped pattern.[3] Both ends of the curve track with higher risk, and the bottom of the bowl sits right around the duration most adults intuitively know they need.
Here is the honest caveat that separates careful reading from scaremongering. The "long sleep is dangerous" half of the U is heavily contaminated by reverse causation: serious illness, depression and frailty make people sleep more, rather than the long sleep causing the illness. The robust, genuinely actionable signal — the one most of us actually control — is the short-sleep side. And that side is well supported. According to PubMed, a separate systematic review, meta-analysis and meta-regression pooling data from more than five million participants found that short sleep was significantly associated with higher mortality (relative risk 1.12) and with markedly increased incidence of type 2 diabetes (1.37), obesity (1.38), coronary heart disease (1.26), cardiovascular disease (1.16) and hypertension (1.17).[4] This is the metabolic and cardiovascular core of the longevity case: chronic short sleep does not just make you groggy, it pushes the very risk factors that shorten lives.
The brain over the long run
If the glymphatic story is the mechanism, the epidemiology of sleep and cognition is the long-term outcome. According to PubMed, an updated 2025 systematic review and meta-analysis of 76 longitudinal cohort studies found that disturbed sleep was a consistent, modifiable risk factor for cognitive decline and dementia: insomnia was associated with a 13% higher dementia risk, while both short and long sleep duration were linked to elevated risk — long sleep carried a relative risk of 1.43 for all-cause dementia and 1.66 for Alzheimer's disease — and poor sleep quality, excessive daytime sleepiness and sleep-related movement disorders all independently raised the odds.[5] The authors' framing matters: they treat sleep as a pivotal modifiable factor, arguing for screening and early intervention rather than fatalism. You cannot change your genes; you can change your sleep.
Sleep, inflammation and the immune system
The thread that connects the heart, the metabolism and the brain is inflammation, and sleep sits at the center of it. According to PubMed, a review in Nature Reviews Immunology describes the reciprocal wiring between sleep and immunity: sleep enhances immune defenses, and signals from immune cells in turn promote sleep, with the relationship running through cytokine responses and the neuroendocrine and autonomic pathways. Crucially, the same review notes that chronic social and psychological stressors drive sleep disturbances that can dysregulate inflammatory and antiviral responses — a low-grade, persistent inflammatory state that is itself a recognized engine of cardiovascular and metabolic aging.[6] This is why bad sleep does not stay in its lane. A disrupted night nudges your inflammatory tone, and inflammatory tone is upstream of almost everything we call aging.
Regularity may matter as much as duration
Most sleep advice fixates on hours. A growing body of evidence says the clock matters too — possibly more. According to PubMed, a prospective UK Biobank study using over 10 million hours of accelerometer data from nearly 61,000 participants calculated a Sleep Regularity Index for each person and found that more regular sleepers had a 20–48% lower risk of all-cause mortality, a 16–39% lower risk of cancer mortality and a 22–57% lower risk of cardiometabolic mortality compared with the most irregular group — and that, head to head, sleep regularity was a stronger predictor of mortality than sleep duration.[7] Your physiology runs on a daily clock; keeping bedtime and wake time consistent, even on weekends, is a free intervention that the data suggest pays off as much as chasing an extra half hour.

What actually improves sleep
None of this requires a gadget. The evidence-based fundamentals are unglamorous and effective: hold a fixed wake time seven days a week, since a consistent wake anchor is the single most powerful input to your circadian clock and the regularity data above. Get morning daylight early, and keep the bedroom cool, dark and quiet. Cut caffeine after early afternoon — it has a half-life of roughly five to six hours, so an afternoon coffee is still circulating at bedtime — and go easy on alcohol near bed, because while it may help you fall asleep, it fragments the deep, slow-wave sleep your glymphatic system depends on. Build a genuine wind-down routine and dim screens beforehand. And for persistent insomnia, cognitive behavioral therapy for insomnia (CBT-I) is the first-line treatment, more durable in the long run than sleeping pills.
The bottom line
You cannot biohack your way around bad sleep, and you cannot supplement your way past it either. Sleep is the substrate on which every other longevity lever — training, diet, mood, even the supplements that are genuinely worth taking — works better or worse. Aim for the bottom of the U at roughly seven to eight hours, protect the deep sleep that runs your brain's cleaning cycle, keep your timing regular, and fix light, caffeine and alcohol before you reach for anything you can buy. Get this one right and almost everything else gets easier.
It is worth pausing on why sleep, of all the longevity levers, is so consistently underrated. Part of it is that the costs of skimping are invisible in the short term — you can run on six hours for years and feel merely "a bit tired," while the damage accrues silently in your arteries, your glucose control and your brain's waste-clearance ledger. Part of it is cultural: short sleep is still quietly worn as a badge of productivity, even though the data treat it as a measurable risk factor on par with the things we take far more seriously. And part of it is simply that nobody profits when you sleep well, so the message has no marketing budget behind it. None of the studies above describe an exotic intervention. They describe the difference between people who protect their sleep and people who do not — measured in years of life and years of healthy brain function. That is an unusually good return for something that costs nothing and that, unlike most of the longevity industry, has decades of human cohort data rather than a single promising mouse.
