Alpha-Ketoglutarate and Aging: Can AKG Really Turn Back Your Biological Clock?

A cheap metabolite from your own cells is the longevity world's newest obsession — and a fresh 2026 study just linked it to a younger biological age. Here's the careful version: the striking numbers, why they need a caveat, and the human trial that will settle it.

Every year or two, the longevity world falls hard for a new molecule. It has been resveratrol, then NAD+ boosters, then taurine. In 2026 a fresh name is climbing the charts: alpha-ketoglutarate, mercifully shortened to AKG. It is cheap, it is something your own body already makes by the gram, and this summer a new study of thousands of dedicated supplement-takers tied it to a measurably younger biological age. The headlines — “reverse aging by years” — wrote themselves. So let's do what the headlines usually don't: look carefully at what AKG is, what the evidence actually shows, and where the honest reader should land.

What alpha-ketoglutarate actually is

Alpha-ketoglutarate (chemical formula C5H6O5) is not some exotic botanical extract. It is a central cog in the Krebs cycle — the chain of reactions inside your mitochondria that turns food into usable energy. Every cell in your body makes and burns through AKG constantly. Beyond energy, it does a surprising amount of side work: it supplies nitrogen for building amino acids, mops up ammonia, acts as a mild antioxidant, and serves as an essential helper molecule for a family of enzymes that regulate the epigenome — the chemical marks that switch genes on and off.[5]

That last job is the one that makes aging researchers sit up. Those same epigenetic marks are exactly what “aging clocks” measure to estimate biological age. And here is the twist that started the whole story: AKG levels are not fixed. They rise with exercise and fasting, and — crucially — they fall as animals get older. A molecule that both powers your cells and helps steer your epigenome, and that drains away with age, is a natural suspect if you're hunting for things that might slow the clock down.

Because pure AKG is chemically unstable, supplements almost always use a mineral salt. The most studied is calcium alpha-ketoglutarate, written Ca-AKG or CaAKG. As we'll see, the exact form turns out to matter more than you'd expect.

The animal result that launched the hype

The serious science began not with humans but with mice. In a 2020 study from the Buck Institute, researchers fed calcium-AKG to aging mice and tracked them for the rest of their lives. The animals didn't just live longer — a roughly 10–12% bump in median lifespan for females — they lived better. The treated mice spent a far larger share of their remaining life healthy and low in frailty, a pattern gerontologists call a “compression of morbidity”: more good years, and a shorter sickly tail at the end.[3]

The team also found a plausible mechanism. AKG appeared to calm the chronic, low-grade inflammation that scientists nickname “inflammaging,” partly by nudging up an anti-inflammatory signal called IL-10. That matters, because a believable biological story is what separates a promising compound from a statistical fluke. This wasn't a fluke: it was a clean, well-run animal study with a coherent explanation attached. The obvious next question — does any of this carry over to people? — is where things get both exciting and murky.

The human numbers — and why they come with an asterisk

The first widely-cited human data landed in 2021. Researchers looked back at 42 people who had been taking Rejuvant, a delayed-release calcium-AKG formula with added vitamins, for an average of about seven months. Using a commercial DNA-methylation age test, they reported an astonishing average drop of roughly 8 years in biological age, with most participants moving in the same direction.[2] Eight years off your biological clock in seven months is the kind of number that sells a lot of bottles.

But read the study design carefully and the excitement has to be tempered. This was a retrospective analysis of people who had already chosen to buy and take the product — there was no placebo group, no randomization, and no control over diet, exercise or the dozens of other things these health-focused users were surely doing. People motivated enough to take a niche longevity supplement are rarely doing only that. With just 42 participants and no comparison arm, an 8-year headline is a hypothesis worth testing, not a proven effect.

This July, a much larger dataset refreshed the conversation. In a 2026 cross-sectional cohort of 4,260 “health enthusiasts” who had taken saliva-based epigenetic age tests, scientists tested 84 common supplements against biological age. Delayed-release calcium-AKG — again the Rejuvant formula — stood out, associated with an average 1.8-year lower biological age even after adjusting for age, sex, smoking and health status.[1] A more grounded number than the original 8 years, drawn from a far bigger group, and it survived statistical correction.

Two details from that 2026 paper deserve emphasis, because they're the kind of thing marketing tends to skip. First, the benefit showed up for the delayed-release Ca-AKG formula specifically; people taking plain, regular AKG showed a much smaller effect that wasn't statistically significant. Formulation appears to matter. Second, the authors were refreshingly candid that their own study can't prove cause and effect. It's an observational cohort, vulnerable to what researchers call “healthy-user bias” — the people who buy premium longevity supplements also tend to sleep, eat and train like people who want to live forever.

Why “biological age went down” is trickier than it sounds

All of these human results lean on aging clocks — algorithms that read DNA-methylation patterns and spit out an estimated biological age. They're a genuine advance, and they've made it possible to study aging without waiting decades for people to die. But they are prediction models, not a tape measure, and a growing literature warns that a single clock reading can wobble, that different clocks disagree, and that a lower number on a test doesn't automatically mean a longer, healthier life.[6]

This is the deeper reason to keep AKG's headlines in perspective. Even if the delayed-release Ca-AKG genuinely nudges a methylation clock, we don't yet know that the nudge translates into what people actually want: more healthy years, fewer diseases, a longer life. The clock is a promising signpost, not the destination — a point the researchers behind these very studies are careful to make.

The trial that will actually settle it

Here is the good news for anyone tired of “more research is needed” hand-waving: the decisive study is already running. It's called ABLE — Alpha-ketoglutarate Supplementation and BiologicaL agE — and it's exactly the design the field has been missing. ABLE is a double-blind, placebo-controlled randomized trial giving 1 gram of sustained-release calcium-AKG or a dummy pill for six months to 120 healthy adults aged 40–60.[4]

The clever part is who they recruited. Rather than take anyone, the team specifically enrolled middle-aged people whose biological age already runs ahead of their calendar age — the group with the most room to improve, and a recruitment feat the investigators confirmed was feasible in a 2025 follow-up report.[7] The primary question is blunt and important: does Ca-AKG actually lower DNA-methylation age versus placebo? Secondary measures include inflammation, grip and leg strength, arterial stiffness and aerobic fitness. Because it's randomized and placebo-controlled, ABLE can do what none of the buzzy cohort studies can — separate a real drug effect from the healthy habits of the people who take it. When it reports, we'll finally have an honest answer.

Safety, dosing and a sensible take

On safety, AKG starts from a reassuring place: it's an endogenous metabolite, something your body already produces and uses in large amounts. Human studies so far, using around 1 gram of calcium-AKG a day, report good tolerability and no serious adverse effects, and reviews of the compound echo that clean short-term profile.[5] The honest caveat is that long-term safety data in healthy people remain thin, simply because the careful trials are only now being done.

So where does that leave a curious reader in 2026? AKG is one of the more intriguing molecules in longevity right now — a compound with a strong lifespan result in mice, a coherent mechanism, and two human datasets pointing the same direction. That's a better starting hand than most supplements can show. But every human number so far comes from observational studies that can't rule out the possibility that healthy people simply take healthy supplements. It is a promising lead under active, rigorous investigation — not a proven anti-aging pill, and certainly not a substitute for the boring interventions with the deepest evidence: regular exercise, decent sleep, not smoking, and keeping your weight and blood pressure in check. If you're tempted to try it, treat it as an experiment with modest expectations, favour the delayed-release calcium form the studies actually used, and keep one eye on the ABLE results — the moment the hype either grows up or deflates.

Common questions

Does alpha-ketoglutarate (AKG) actually reverse biological age?

The evidence is promising but not yet proof. A 2021 retrospective analysis of 42 people taking the AKG-based formula Rejuvant reported an average 8-year drop on a DNA-methylation age test, and a 2026 study of 4,260 supplement users linked delayed-release calcium-AKG to a 1.8-year lower biological age. But both were observational, not placebo-controlled, so they cannot prove AKG caused the change. What is the difference between AKG and Ca-AKG?

AKG (alpha-ketoglutarate, C5H6O5) is a natural metabolite in the Krebs energy cycle. Because pure AKG is unstable, supplements use a mineral salt for stability, most often calcium alpha-ketoglutarate (Ca-AKG or CaAKG). Products like Rejuvant use a delayed-release Ca-AKG so it survives the stomach. Notably, the 2026 cohort found the biggest signal for the delayed-release Ca-AKG formula and only a small, non-significant effect for plain AKG.

Is alpha-ketoglutarate safe to take?

AKG is a molecule your own cells make, and human studies to date report a good tolerability profile with no serious side effects at doses around 1 gram of calcium-AKG per day. That said, long-term safety data in healthy people are still limited and the strongest efficacy claims rest on uncontrolled studies.
