Biological Age Reversal: What the Science Really Shows

Clinics charge five and six figures to “turn back your biological clock,” and a new diet study says you can shave years off in a month. Here's the careful version — what the real trials found, and where the word “reversal” runs ahead of the evidence.

Somewhere in Florida right now, someone is paying a longevity clinic to tell them they're younger than their driver's licence says. “Biological age reversal” has become one of the hottest phrases in wellness — the subject of a reported $50-billion industry, glossy clinics charging anywhere from a few hundred dollars to more than $150,000 a year, and an endless scroll of before-and-after clock readings on social media. Then, in mid-2026, a University of Sydney study handed the trend its perfect headline: older adults who tweaked their diet for just four weeks looked measurably “younger” on a biological-age score. The internet did the rest.

So is age reversal real, or is it the most sophisticated marketing story in health? The honest answer sits in an uncomfortable middle. The underlying science is genuine and fascinating. The word “reversal,” as it's usually sold, is doing a lot of heavy lifting. Here's what the evidence actually supports.

What “biological age” even means

Your chronological age is just how long you've been alive. Your biological age is an attempt to measure something more useful: how worn-down your body actually is, and how fast it's heading toward disease. Two 60-year-olds can be a decade apart biologically — one with the arteries, immune system and cellular housekeeping of a healthy 52-year-old, the other of a struggling 68-year-old. Capturing that difference in a single number is the whole game.[4]

The most talked-about way to do it is the epigenetic clock. As you age, your DNA accumulates a shifting pattern of chemical tags — mostly methyl groups (a small –CH3 cluster) that attach to specific spots on the genome and switch genes up or down. These tags don't change the DNA code itself, but their pattern drifts in a surprisingly predictable way over a lifetime. In 2013, researcher Steve Horvath showed you could read that pattern and estimate a person's age to within a few years. Newer “second-generation” clocks — with names like PhenoAge, GrimAge and DunedinPACE — go further, and are tuned to predict not just age but future illness and death.[4]

The reversal boom — and why to keep your wallet in your pocket

Once you can put a number on aging, you can sell people a smaller one. That's the engine behind the longevity-clinic boom. Walk in, give blood or saliva, and walk out with a report saying your body is “really” 47, plus a menu of peptides, infusions, supplements and coaching promising to drive that figure down. The appeal is obvious. The problem is that a lower clock reading is not the same thing as a longer, healthier life — and almost none of the bundled protocols on offer have been tested in a controlled trial to see if they do anything beyond moving a lab value.

It's worth being blunt here, because this is exactly where the money is. An epigenetic age estimate can shift with something as trivial as a cold, a poor night's sleep, or which lab ran the sample. Treating a single reading as a verdict — and then paying five figures to “fix” it — is running ahead of what the science can currently back up. What the strongest human trial actually found

Here's the study the marketing rarely mentions. The CALERIE trial randomised 220 healthy adults to either cut their calories by about 25 percent or eat normally, for two full years — a serious, controlled test of whether a lifestyle change can move the aging clock. When researchers later analysed the DNA-methylation data, they found something genuinely important but carefully bounded: calorie restriction slowed the pace of aging as measured by the DunedinPACE clock. It did not significantly change the PhenoAge or GrimAge estimates — the clocks most tightly linked to disease and death — and even the effect it did find was small.[1]

That result is the honest high-water mark for human evidence. Read it carefully and it says two things at once. First, aging really is modifiable — a controlled trial nudged a validated clock with nothing more exotic than eating less, which supports the whole premise. Second, what it moved was the rate of aging, slightly, not a rewind of the clock. Slowing how fast you age is a real and valuable goal. It is also a very different promise from the one on the clinic brochure.

The four-week diet study everyone shared

Now the viral one. In 2026, University of Sydney researchers ran a tightly controlled feeding trial — the Nutrition for Healthy Living study — on 104 adults aged 65 to 75. Everyone ate one of four diets for four weeks, varying in protein source (omnivorous versus semi-vegetarian) and in fat versus carbohydrate. The team then calculated each person's biological age using a composite biomarker index, and looked at how the gap between biological and chronological age changed.[2]

The finding that lit up the headlines: compared with a high-fat omnivorous diet closest to people's normal eating, the groups that shifted toward more carbohydrate or less animal protein showed a measurable drop in their biological-age gap — in some cases the equivalent of looking a few years younger, in just a month. It's a striking result, and a real one.

But the most important sentence in the paper came from the authors, not the press release. They cautioned directly against reading the change as biological age reversal, noting the shift “may reflect acute physiological responsiveness to dietary inputs rather than altered ageing trajectories.”[2] In plain English: change what someone eats and their blood chemistry moves within days — blood sugar, lipids, inflammation markers — and a biomarker-based age score moves with it. That's your body responding to lunch, which is not the same as your body becoming younger. Whether these short-term shifts translate into fewer diseases years later, the authors say, needs longer trials to know.

The one study that used the word “reversal”

You may have seen the claim that epigenetic age has already been reversed in people. It traces back to a small 2019 study called TRIIM, in which nine men took a cocktail aimed at regenerating the thymus gland for a year. On four different epigenetic clocks, their estimated ages ended up around two and a half years lower than where they started — a headline-grabbing apparent rewind.[3]

It's a genuinely intriguing result, and worth taking seriously. It's also exactly the kind of study you should not build a health decision on. Nine participants. No control group. All men. A complex drug mix that included growth hormone, with real potential side effects. A finding like that is a reason to run a bigger, controlled trial — which is now happening — not a reason to conclude the problem is solved. The gap between “nine people's clocks read lower” and “you can reverse human aging” is enormous, and honest science lives inside that gap.

So can you actually turn back your clock?

Put the evidence together and a clear, unglamorous picture emerges. Biological age is a real and useful concept. The clocks that measure it are impressive science but still noisy, lab-dependent, and not ready to guide medical decisions on their own. Controlled trials show that ordinary lifestyle change can slow the pace of aging by a modest amount. Short studies can move a biomarker score quickly, but at least partly because your body responds fast to what you eat and do. And the one human study that genuinely showed a “reversal” was tiny and uncontrolled.

The practical takeaway is almost anticlimactic, and that's the point. The levers that reliably move biological-age markers in the right direction are the same boring, powerful ones that have topped the evidence for decades: regular exercise (especially anything that builds cardiorespiratory fitness), good sleep, not smoking, keeping a healthy weight, and a diet weighted toward plants, fibre and adequate but not excessive protein. Several of the interventions with the best human aging-clock data — from a well-run omega-3 trial to newer signals around GLP-1 medications — nudge the same dials, modestly, on top of those basics.

None of that reverses time. What it does is something more realistic and, frankly, more valuable than a clinic's promise: it slows the rate at which you age and stacks the odds toward more healthy years. If a lower number on a screen helps you stick to those habits, use it as motivation. Just don't confuse the number for the goal — or pay six figures chasing it.

Common questions

Can you actually reverse your biological age?

You can measurably lower some markers of biological age, but “reversal” is a bigger claim than the evidence supports. Randomised trials show lifestyle changes such as calorie restriction can slow the pace of aging on certain DNA-methylation clocks, and short diet studies can shift composite biomarker scores within weeks. But those shifts may partly reflect temporary physiological responses rather than a permanently younger body, and the largest controlled trial found no change on the clocks most tied to disease and death. The honest position: you can nudge the dials, but proven, lasting age reversal in humans has not been demonstrated.

What is the best test to measure biological age?

There is no single validated “best” test. The most studied are second-generation DNA-methylation clocks such as PhenoAge, GrimAge and DunedinPACE, which predict morbidity and mortality better than earlier clocks. But results vary between labs, can shift with a cold or a bad night's sleep, and are not yet used to guide medical decisions. Simpler composite scores built from routine blood markers, blood pressure and fitness are cheaper and track health reasonably well. Treat any at-home biological-age number as a rough snapshot, not a diagnosis.

Do longevity clinics that promise age reversal work?

Longevity clinics offering biological-age testing, peptides, infusions and other therapies are a fast-growing and largely unregulated industry, with prices ranging from a few hundred to well over a hundred thousand dollars a year. Very few of their bundled protocols have been tested in controlled trials, and a lower clock reading after treatment does not prove reduced disease risk. The interventions with the strongest evidence — exercise, sleep, not smoking, a healthy weight and diet — are mostly free and available without a clinic.
