How to Raise Your VO2 Max: Zone 2 Cardio, Intervals, and What Actually Works

The short version

Most longevity advice starts with something you should swallow. This one starts with how hard you can breathe. VO2 max — the maximum amount of oxygen your body can take in and use during all-out effort — keeps showing up in research as one of the strongest single predictors of how long a person lives. It's not a supplement or a blood marker you can tweak overnight. It reflects how well your heart, lungs, blood, and muscles all work together under real physical load.

That might sound like the kind of overstatement longevity content is famous for. So let's hold it to a higher standard: what does the evidence actually show, where does it fall short, and what can you realistically do about it?

The number that beat smoking

The clearest large-scale evidence comes from the Cleveland Clinic. Researchers followed 122,007 adults who had taken a treadmill fitness test, tracking them for a median of 8.4 years.[1] When they ranked people by fitness level and accounted for other health factors, the pattern was striking: the least-fit group had roughly five times the risk of dying during follow-up compared with the fittest group (adjusted hazard ratio 5.04). To put that in context, being unfit was associated with a bigger mortality penalty in this group than having heart disease, diabetes, or a smoking habit.

What made this study stand out is what happened at the top end. There was no ceiling. The very fittest people — those more than two standard deviations above average for their age and sex — still had lower death rates than the group just below them. In the world of longevity research, "more is better with no plateau" is a rare and powerful finding. It even held up in people over 70 and in those with high blood pressure.

Why this isn't proof — and why it still matters

Here is the caveat most articles skip. This was an observational study, not a randomized trial — nobody was randomly assigned to be fit or unfit. That opens the door to reverse causation: people with hidden, early-stage illness often score lower on a fitness test precisely because they are already getting sick. Some of the "low fitness leads to early death" signal almost certainly reflects "undetected illness causes low fitness." Fit people also tend to smoke less, sleep better, and generally have more access to good healthcare — all factors the statistics try to account for, but can never fully untangle.

So this data alone cannot promise that improving your fitness from average to excellent will personally add years to your life. What it does show is that the link is enormous, consistent across many separate studies, biologically believable, and follows a clear dose-response pattern — more fitness, lower risk, smoothly, all the way up.[2] When an association is that strong and that consistent, it becomes harder to dismiss. And fitness is one of the few risk factors on this list you can genuinely change.

What VO2 max really measures

It helps to understand why this number carries so much information. VO2 max is a test of an entire chain of systems working together: your heart's pumping power, the oxygen-carrying capacity of your blood, the network of tiny blood vessels feeding your muscles, and the cellular engines inside those muscles — called mitochondria — that actually burn the oxygen. A high VO2 max means all of those systems are healthy and connected. A low one is a quiet warning sign that something in that chain — your heart output, blood vessel health, or muscle metabolism — has started to slip. That's why it tells you things a single blood test cannot.

VO2 max also naturally declines with age — roughly 10 percent per decade after your 30s if you do nothing. The practical takeaway is sobering: someone who builds strong aerobic fitness in midlife is buying themselves a buffer. Starting from a higher peak, the same gradual decline keeps them above the threshold of physical frailty for many more years.

There's a second reason this number matters, and it has nothing to do with your final years. Frailty, loss of independence, struggling to climb stairs or carry groceries — these are all downstream of the same aerobic fitness. A treadmill test isn't measuring some abstract longevity score; it's measuring whether, decades from now, you can still do the everyday things that make life feel like your own. The mortality data gets the headlines, but quality of life is arguably the more honest reason to care.

Zone 2: the unglamorous engine

This is where Zone 2 training comes in. Zone 2 is steady, easy-paced cardio — an effort where you can still hold a conversation, but talking takes a little focus. It sits just below the point where your body starts struggling to clear lactic acid quickly. It doesn't feel like a hard workout — honestly, it feels almost too easy. But over time it does something important: it grows more tiny blood vessels around your muscles and builds more mitochondria, which is the foundation that a higher VO2 max is built on.

The reason coaches recommend Zone 2 as the cornerstone of aerobic training is volume. You can rack up three to four hours a week of it without needing days to recover, unlike hard interval sessions. Think of it as making regular deposits into a slowly compounding account, rather than chasing one big win.

The case for occasional intervals

Zone 2 builds the base, but truly pushing your VO2 max higher also needs a small dose of genuine intensity. A review of 28 controlled trials found that both steady endurance training and short bursts of high-intensity effort (interval training) produced big gains in VO2 max in healthy adults — with interval work delivering a modest extra edge of about 1.2 mL/kg/min over endurance training alone.[3] And this isn't only for younger people. A 2024 review of 29 trials in adults over 60 found that both interval and steady moderate training improved aerobic fitness, with intervals showing an advantage in the better-designed studies.[4] A separate meta-analysis pooling interval versus moderate continuous training specifically in middle-aged and older adults reached the same verdict — interval work produced a significantly greater VO2 max gain, roughly 1.1 mL/kg/min more.[6] The flip side, from a meta-analysis in women, is reassuring for anyone who dislikes hard efforts: moderate continuous and interval training raised VO2 max about equally, and the biggest predictor of improvement was simply doing more sessions.[7]

In practice, "intervals" doesn't mean punishing yourself every day. A common, research-backed approach is to keep the bulk of your weekly cardio in Zone 2, and add one harder session — for example, four four-minute efforts near your maximum, with recovery breaks in between. The hard sessions raise the ceiling; the easy volume builds the floor that lets you absorb and benefit from them. One genuinely hard session a week, done consistently for months, moves the number more reliably than three half-hearted ones.

How much can you actually change it?

This is where honesty matters again, because people respond to training very differently. The HERITAGE Family Study put sedentary adults through the exact same 20-week exercise program and measured what happened. Gains in VO2 max ranged from almost nothing to over 600 mL/min, and the researchers estimated that about 47 percent of that variation came down to genetics.[5] In other words, your genes partly determine how strongly your body responds to the same training. Some people are high responders; others, frustratingly, are not.

But — and this is the part worth holding onto — even low responders improved, and where you start matters more than how fast you climb. The biggest absolute gains across the research go to people who begin unfit. If you are sedentary right now, you are sitting on the steepest part of the improvement curve. The first few months of consistent aerobic training typically deliver the biggest fitness jump you will ever get, regardless of your genetics.

It's also worth being clear about what the genetics research does not mean. A modest genetic limit on how high your VO2 max can climb is not the same as a limit on the health benefits of training. Even people whose numbers barely budge still gain the metabolic and cardiovascular rewards of regular exercise. The point of the HERITAGE work isn't to discourage anyone — it's to set realistic expectations, so a slow responder doesn't quit in month three thinking the program has failed when it's actually working exactly as it should.

What to actually do

The practical plan that emerges from all of this is simple and sustainable. Build two to four hours a week of Zone 2 cardio — brisk uphill walking, cycling, rowing, easy jogging, anything that holds a steady, conversational effort. Once that feels comfortable, add one weekly session of harder intervals. Show up consistently over months and years, not heroically for three weeks. And keep in mind that the relationship between effort and fitness works in both directions: doing more, sensibly, keeps moving the needle even when other health markers have leveled off.[2]

VO2 max isn't magic, and no dataset can personally guarantee you a longer life. But of all the numbers people chase in the name of longevity, this one is among the best-supported by evidence, most tightly linked to survival, and — unlike your age or your DNA — something you can genuinely improve. That combination is rare enough to take seriously. You don't need a lab test to get started. You just need to breathe hard, on purpose, most weeks, for the rest of your life.
